Talkspace pairs the subscription-therapy format (live sessions plus messaging) with two things betterhelp doesn't have: psychiatry with medication management, and insurance acceptance. Many major insurance plans and employee-assistance programs cover Talkspace, which can turn therapy from a significant monthly out-of-pocket cost into a copay - for eligible plans, that changes the affordability math entirely. Talkspace says most insured members pay a $0 copay - its own claim, but one your eligibility check confirms or denies in minutes - and the app-based format means 24/7 access to your therapy room, not just session slots. It also carries the strongest verified customer rating in our online-therapy coverage: 4.4 on Trustpilot across 2,370 reviews (ahead of betterhelp's 3.9). Coverage is plan-specific, so run its eligibility check with your insurance details before assuming anything. We haven't verified Talkspace's current self-pay rates, so check those on its site if you're paying out of pocket.

Which online therapy platform offers psychiatry?

Talkspace is the only platform in our coverage with psychiatry on-platform: an initial evaluation with a psychiatric clinician by video, a prescription sent to your local pharmacy where appropriate, and follow-up visits for medication management - alongside its therapy service, so a therapist and prescriber can stay coordinated. It's in-network with many major insurance plans, and insurance can apply to psychiatry plan-permitting. betterhelp is talk therapy only and headspace's app is self-guided practice, so with those a prescriber is a separate provider.

How does online psychiatry work?

The same rhythm as office-based psychiatry, over video:

  • An initial evaluation - longer than a routine visit - where the clinician reviews your history and symptoms and works toward a diagnosis.
  • A prescription to your local pharmacy, if medication is appropriate; common medications for anxiety and depression are routinely handled online.
  • Follow-up visits to check how you respond and adjust the dose or plan - the part a good service earns its keep on.
  • Limits: certain controlled substances face stricter telehealth rules and may require in-person care. A responsible service says so up front.

Does insurance cover online psychiatry?

Often, but coverage for psychiatry and coverage for therapy can differ on the same plan, so run Talkspace's eligibility check with psychiatry in mind. For self-pay, psychiatry is priced separately from therapy - typically an initial-visit price and a follow-up price; we haven't verified current rates, so the live numbers are on Talkspace's site. Our Talkspace cost guide and insurance guide go deeper.

Do I need a psychiatrist or a therapist?

A reasonable rule: if you want to work through thoughts, patterns, relationships or life stressors, talk therapy is the natural start. If symptoms are significantly disrupting daily functioning - or a therapist or doctor suggests medication might help - that points to a psychiatric evaluation. Many people use both, and you don't have to decide perfectly in advance; many start with therapy and are referred onward. Our guide to therapist vs psychiatrist covers the distinction, and online psychiatry explains how to spot a legitimate service.

How we chose

We looked for a real evaluation with a licensed psychiatric clinician, genuine ongoing follow-up, honest limits about what can be prescribed remotely, and coordination with therapy on the same platform. Only one platform in our coverage meets that bar today; we'll add others as partnerships with psychiatry-capable services come online. See the full field on our online therapy comparison.
